why isn't the call using the call forwarding code initiated automatically? i don't mind pressing the call button, but having to copy and paste the code over is just too tedious for day-to-day use.

Not very possible - iphone has several limitations.

As it is written everywhere (in the app, in app description, in the info page, in the web manual etc), deleting the app WILL NOT reset your forwarding. Just call the generated reset code.
